Chief Complaint / Presenting Problem

Non-healing wound on right sole for 2 months, spreading infection with foul smell. Referred from rural hospital for possible amputation.

Diagnosis

Diabetic Foot Ulcer, Grade IIIB (Wagner), Osteomyelitis Calcaneum, Peripheral Neuropathy

Treatment & Procedure

Strict glycaemic control, IV antibiotics based on culture sensitivity, surgical debridement and sequestrectomy. Wound care with Negative Pressure Wound Therapy (NPWT). Vascular surgery for revascularisation.

Outcome & Result

Wound healed completely at 14 weeks with limb salvage. HbA1c brought down from 11.5% to 7.2%. Foot care and footwear education. Zero amputation achieved.
